Step 1: Initial Consultation and Needs Assessment


The process begins with an in-depth consultation, during which we meet with the individual and their family or guardian to assess their unique goals, abilities, and support needs. This step helps us understand the individual's strengths, challenges, and what they hope to achieve through Day Support services.

Step 2: Development of a Personalized Support Plan


We create a tailored support plan in collaboration with the individual, their family or guardian, clinical supervisor, and state-assigned care coordinator. This plan outlines specific goals, including skill development, social interaction, and community engagement, ensuring that the services provided align with the individual’s aspirations for independence and well-being.

Step 3: Introduction to the Day Support Program


We schedule an assessment day or trial day of the program, which may include group activities, task-based learning, and therapeutic sessions. This allows you to decide if the program is a good fit for you and your loved one. We ensure a smooth transition into the program by providing constant support and supervision.

Step 4: Skill Development and Community Integration


Day-to-day activities are focused on helping the individual build key life skills. Community integration is a central part of this phase, where individuals participate in volunteer programs, social events, and other activities that promote social inclusion and real-world skill application.

Step 5: Ongoing Monitoring and Adjustments


As the individual progresses, their clinical supervisor and the Amivie team continuously monitor their development. Regular check-ins with the individual and family allow us to assess how well the support plan is working and make any necessary adjustments to ensure the individual continues to grow and thrive.

Paying for Your Care

To qualify for day support, individuals must meet the following criteria:

  • Qualifying IDD Diagnosis: The individual must have a documented intellectual or developmental disability diagnosed before the age of 22.
  • Proof of Diagnosis: A psychological assessment with IQ and adaptive behavior scores is required.

Each state has its own process for determining eligibility for services, and funding options may vary. Day Programs are typically funded through Medicaid or state-specific waiver programs. Private pay is an additional funding option. The rate is determined by state Medicaid non-compete rates.
