How Residential Services Work

Step 1: Initial Needs Assessment
We start with a thorough assessment of the individual’s unique needs, goals, and preferences, conducted with the individual, their family, and other support team members to fully understand what they are looking for in a residential setting.

Step 2: Personalized Care Plan Development
Based on the assessment, our team creates a tailored care plan that outlines the specific support services, skill-building activities, and community involvement opportunities that will best meet the individual's needs.

Step 3: Residential Placement Matching
We use a personalized matching process to pair the individual with a compatible DSP and determine the ideal residential environment, ensuring the placement aligns with their personality, needs, and interests.

Step 4: Move-In and Orientation
Once matched, the individual moves into their new home, where they receive a warm orientation to introduce them to house routines, nearby resources, and available activities, helping them feel comfortable and welcomed.

Step 5: Ongoing, 24/7 Support and Skill-Building
DSPs provide 24-hour assistance, helping individuals build essential life and community skills, manage daily tasks, and pursue personal goals, with progress monitored and adjustments made to the care plan as needed.

Step 6: Regular Progress Evaluations and Family Updates
We conduct routine evaluations to ensure the care plan remains aligned with the individual’s evolving goals. Families receive regular updates on their loved one’s progress and have opportunities to discuss any adjustments.

Paying for Your Care

To qualify for group residential services, individuals must meet the following criteria:

  • Qualifying IDD Diagnosis: The individual must have a documented intellectual or developmental disability diagnosed before the age of 22.
  • Proof of Diagnosis: A psychological assessment with IQ and adaptive behavior scores is required.

Each state has its own process for determining eligibility for services, and funding options may vary. Group homes are typically funded through Medicaid or state-specific waiver programs.
