Step 1: Initial Needs Assessment


Amivie begins with a comprehensive needs assessment, meeting with the individual, family members, and any caregivers to understand the individual's goals, preferences, and support requirements.

Step 2: Personalized Care Plan Development


Based on the assessment, Amivie develops an individualized care plan tailored to the person’s unique needs. This plan outlines the types of support the individual will receive, including specific services such as day programs, employment support, or behavioral interventions, with measurable goals to track progress.

Step 3: Direct Support Professional Matchmaking


Amivie uses a thoughtful matchmaking process to pair the individual with a DSP who aligns with their personality, interests, and care requirements. This helps create a comfortable and trusting environment that supports meaningful connections and consistency.

Once matched with a DSP, services begin according to the individualized care plan.  Amivie staff work closely with the individual and their family during the initial stages to ensure a smooth transition and provide guidance on routines, goals, and expectations.

Our supervisors conduct regular check-ins and assessments to monitor progress and ensure the individual’s needs are being met. Any necessary adjustments to the care plan are made in collaboration with the individual, family, and DSP to adapt to evolving needs and goals.

Amivie provides regular updates to families, guardians, or care teams, sharing progress and discussing any adjustments to the service plan. This collaborative approach ensures everyone involved stays informed and aligned on the individual’s care and progress.

Paying for Your Care

To qualify for day support, individuals must meet the following criteria:

  • Qualifying IDD Diagnosis: The individual must have a documented intellectual or developmental disability diagnosed before the age of 22.
  • Proof of Diagnosis: A psychological assessment with IQ and adaptive behavior scores is required.

Each state has its own process for determining eligibility for services, and funding options may vary. Day Programs are typically funded through Medicaid or state-specific waiver programs. Private pay is an additional funding option. The rate is determined by state Medicaid non-compete rates.
